This qualitative, particularistic case study explored the perceptions of case management professionals related to their experiences of successful transition from practitioner based patient centered care to leadership positions on non-profit professional membership or trade organizations' boards of directors. The context for the study is a professional membership association for case managers in health care. Data were collected through recorded individual interviews, surveys, and focus groups. Data were analyzed through an analytical strategy based on the theoretical propositions. The specific focus of the study is reflected in the research questions, which concerned leadership styles or traits as identified in professional membership associations and training and transition of new board members as related to the future of NPO. Data analysis findings revealed four themes: (a) skill sets or traits for transition; (b) Service to others; (c) passion and integrity for the profession; and (d) orientation and preparedness for leadership. Recommendations to leaders included: servant leadership as a viable leadership model; programs in formal orientation, training, mentoring program; and strategic planning for leadership development and sustainability of NPOs. Recommendations for future research include expanding the study to include board member leadership and orientation in other professions or a quantitative study developing a formal orientation model or mentorship program for effective board member orientation. A quantitative research study may also help to determine if any one specific trait is more indicative of successful transition,
